Expense Subsidy Application Process

PLEASE READ THIS BEFORE COMPLETING THE EXPENSE SUBSIDY APPLICATION FORM

The 2010 Forum on HIV and Rehabilitation and CWGHR annual general meeting (AGM) will be held June 12 - 13, 2010 in Toronto.

CWGHR is able to provide a limited number of expense subsidies to assist some members with the costs of attending the Forum and AGM.

Please note:

  1. The deadline for expense subsidy applications is April 1, 2010 at 5:00 p.m. ET.  The completed application form must be received (by mail, fax or on-line registration) by this date at the CWGHR offices in Toronto. The address is on the enclosed application form.
  2. Expense subsidies are only available to full (voting) CWGHR members in good standing.
  3. Expense subsidy recipients are expected to attend all events on June 12 - 13.
  4. If an applicant is not an individual member of CWGHR , but is affiliated with an organization that is a member, an expense subsidy may only be provided if the applicant will be representing that organization as its official delegate to the AGM. The expense subsidy application form must be signed by the organization’s executive director or board chair. Only one expense subsidy application form may be submitted per member organization.
  5. As funds are very limited, CWGHR will not likely be able to grant any full expense subsidies; we will be able, however, to provide some partial allowances to offset the costs of travel, accommodation, per diem and/or registration fees.
  6. Expense subsidies will be awarded to eligible members at a maximum dollar value based on the information provided by the member.
  7. Expense subsidies will be awarded by a committee of CWGHR staff and board of directors, based on the following considerations:
    • The amount of the subsidy requested;
    • The number of applications received and the funds available;
    • Whether the applicant is self-identified as living with HIV/AIDS or (other) disability;
    • Whether the applicant or their organization is a full voting member of CWGHR;
    • Whether the work of the applicant or their organization involves groups with specific needs and/or from underrepresented regions;
    • Whether an award will ensure a balance of regional representation, as well as in rehabilitation and HIV/AIDS fields;
    • Subsidies will be given to those with demonstrated commitment to HIV/AIDS work;
    • Applicants must be a registered member of CWGHR at the time they apply;
    • Subsidies will be granted to only one representative from each organization.
